Who we work with
This page is intended for case managers, social service providers, veteran service organizations (SSVF, GPD, VA-connected programs), county and nonprofit housing programs, and re-entry, victim services, and community-based organizations. If you support individuals who need short-term stabilization or a structured transitional housing option, we welcome the opportunity to connect.
Our program options
We offer a range of specialized, structured housing services designed to support individuals in critical transition periods. Each option focuses on stability, accountability, and connection to community resources.
30-day emergency stabilization
A short-term option for immediate stability, ideal for individuals transitioning from shelters, hospitals, or crisis. Supports initial stabilization while next steps are determined.
Transitional housing (up to 12 months)
A longer-term, structured environment focused on accountability, tracking progress and connecting to employment, benefits, and community supports for independence.
Our program-based housing model
Furnished residences with clear expectations, a drug-free, violence-free environment, and safety measures. Participants enroll in a program, not a tenant-landlord relationship, ensuring stability.

How referrals work & our partnership approach
Our referral process is straightforward- Referring agencies may contact SmartHaven by email with information about a potential participant or submit a referral form. We then connect for a brief conversation to determine fit and timing, and placements are coordinated based on availability.
We take a flexible, collaborative approach to partnerships and are open to a variety of models, including simple referrals, small pilot placements, and cost-sharing support when available. We believe strong partnerships expand housing options while supporting and strengthening the work already being done by service providers.
Veteran-specific partnerships
SmartHaven welcomes collaboration with veteran service organizations, including SSVF and GPD programs. Our housing options may support veterans needing short-term stabilization (30-day emergency option) and those working toward longer-term housing stability (transitional housing up to 12 months). We are open to referral coordination, small pilot placements, and cost-sharing support when available. We recognize the importance of structure, accountability, and clear communication when serving veterans and aim to align with partner program goals.
